We like to make checking in for your first appointment as easy as possible. Before your appointment, we will send you an email to fill out, which will then upload directly to us. This makes it easy, so you don’t need to bring any paperwork with you to your appointment!
Please make sure to bring your dental insurance card and a list of your current medications. If possible, have your previous dentist send your dental record and recent dental x-rays to our office.
